Your Sirv partner is local. That means care is planned, delivered and managed by people in your community, backed by a national team for quality, compliance and safe care.
"“We're changing the way in-home aged care services are delivered and managed across Melbourne and VIC…. We serve you.”
Sirv Melbourne (North & West) is a local in-home aged care team led by Danica De Leon, a Registered Nurse with over 13 years’ experience across hospital, community and aged care settings. We help older people live safely, independently and confidently in their own homes across the North and West of Melbourne.
Danica’s background in clinical care, care management and leadership gives our team a deep understanding of how important it is for older people and their families to have care that is reliable, genuinely personal and responsive to their needs.
Our focus is on building meaningful relationships with clients, families and the wider community, while making sure every person receives high-quality, compassionate and person-centred support. We believe great care starts with listening creating a service where people feel heard, respected and supported, and where families can have confidence their loved ones are in caring, capable hands.
Our business unit brings together strong clinical knowledge, aged care expertise and a genuine commitment to person-centred care. With extensive experience across nursing, community care, care management and clinical leadership, we understand what it takes to provide safe, reliable, high-quality support tailored to each individual. We have particular expertise in supporting people with complex care needs coordinating services, providing clinical oversight, and helping people and families navigate the aged care system.
We’re passionate about helping older people live independently, safely and with dignity in the comfort of their own homes. We believe care should be personal, compassionate and built around what matters most to each person creating genuine connections with our clients and families, listening to their needs, and providing support that promotes choice, confidence and quality of life.
"Grace speaks very highly of her support worker Sam and is really enjoying her supports with Sirv, due to the great match the managing partner, Samantha Lovett, has provided. Grace said she thinks that the supports are going well because Samantha took the time to match her with a support worker that would work well with her. Please thank Sam for doing an amazing job and having a positive impact in our participant's life!"
"Bron brings a wealth of knowledge to her role as Managing Partner with Sirv. Her passion to assist NDIS participants to live a full and meaningful life is clearly demonstrated by her dedication and quick-to-solutions attitude. Aided by her years of experience, Bron demonstrates person-centered practices and a holistic approach to NDIS supports and services. As a local Support Coordinator, I have the upmost confidence in referring the people I support to providers like Sirv."
"Sirv is the best service provider I have encountered. Others tried but gave up. Many thanks to your team of workers for making a big difference!"
"I wanted to provide some positive feedback on the Sirv support staff at the residence at Towradgi (…). All of the staff are friendly and professional whenever I attend the house"
"Matt (Sirv staff) engages with Trev (participant) in a friendly and kind manner and has taken the time to get to know Trevor's interests. He connects with him on his level, never condescending or frustrated by communication challenges. Matt went above and beyond by customising Trev's iPad communication app, a task typically handled by a speech therapist, to help him connect more easily with house staff."
"Yasmyn's communications with Trev along with her openness and kindness make her an excellent support for Trevor. Her genuine concern for Trev after he received the news from myself that his only sibling (his brother) had passed away overseas was reassuring for me to see. She knows Trevor well and treats him with dignity and respect."
"Since working with Sal, our daughter has not only reduced her meltdowns but is now making eye contact and using words to express frustration. It's changed our family dynamic."
"The connection-first model gave me tools to build trust with my students. Our class culture has shifted dramatically."
"Bettina is fantastic – thank you so much... Hunter honestly had the best day yesterday, I feel like crying as it's been very challenging in finding the right fit for him!"

Support at Home is the Australian Government's home care program for older people. It started on 1 November 2025, replacing the Home Care Packages program and the Short-Term Restorative Care programme. It's designed to give you coordinated, personalised support so you can keep living safely and independently at home.
Everything begins with an aged care assessment, arranged through My Aged Care. Once you're approved, you'll be given a funding level based on your needs. From there, you choose a registered provider such as Sirv to help you make the most of that funding and put your support in place. It's worth choosing one who's local, easy to reach, and known to deliver quality care in your community.
It can cover a wide range of everyday and clinical support, from help around the home and getting out and about, to nursing and allied health care. What you're approved for depends on your individual support plan, based on your assessed needs. Our local Sirv team can help make sure every part of your plan is put to good use.
Some everyday services may involve a personal contribution based on your income and assets, while clinical care is government-funded. The government has indicated some contribution arrangements are changing during 2026, so the exact amount you'd pay is best confirmed for your own circumstances through My Aged Care. If you'd like it explained in plain terms, Sirv's team is happy to talk it through with you, no obligation.
Our team can walk you through the assessment process, help you understand what your funding can be used for, and take care of the arrangements so you can focus on the support itself, not the paperwork. Find your local Sirv team here and reach out today!
