“We're changing the way disability & aged care support services are delivered and managed across Fraser Coast and QLD….

We serve you.”

Why choose our local Fraser Coast team

Sirv Fraser Coast is a local disability and aged care support team led by Shelley Munford, a Registered Nurse committed to helping people recognise their strengths and build a more purposeful, fulfilling life. We deliver safe, holistic, person-centred care to people living with disability and to older Australians across the Fraser Coast, from Gympie through to Bundaberg.

Our Expertise

As a Registered Nurse-led service, we bring the clinical knowledge and experience to recognise and respond to each person’s healthcare needs, delivering safe, holistic and person-centred care. Working within the community lets us build meaningful relationships and empower people to live independently, maintain their wellbeing and lead fulfilling lives.

Our Passion

We’re committed to delivering safe, high-quality care that empowers people to build on their own strengths and abilities, with access to the right supports and services. Respecting each person’s goals, beliefs and values is central to how we work, with a strong focus on holistic, person-centred care that supports overall wellbeing.

Our Capability

I pride myself on providing a respectful, compassionate service built on collaboration and trust. By working in partnership with each client, I ensure they remain at the centre of every decision and all aspects of their care. Achieving positive outcomes through person-centred care has always been my primary focus. Where appropriate, I collaborate with allied health professionals and other members of the care team to deliver coordinated, safe, and high-quality care tailored to each individual's unique needs.

The FAQ’s

Yes. Sirv is a registered NDIS provider, which means we meet the NDIS Practice Standards and are independently audited for quality and safety. You can use our supports whether you're plan-managed, self-managed or NDIA-managed.

Our local team can support you with everyday things like personal care, help around the home and getting out into the community, right through to Supported Independent Living (SIL), 24/7 support and more complex or high-needs care. We build your supports around your goals, not a fixed menu.

For NDIS-funded supports you'll need an approved plan with funding in the relevant category. If you're not sure what your plan covers, or you're still waiting on one, reach out anyway — we can talk you through it, and we also offer privately funded support so you don't have to wait.

We take the time to understand your needs, routines and preferences, then match you with a support worker who genuinely fits. Because your partner is local, that match is made by someone who knows the workers and knows your community.

Absolutely. Your comfort matters most. If a match isn't quite right, just let your local team know and we'll arrange an alternative.

Supports are delivered locally by your Managing Partner's team, and coverage is location-specific. The quickest way to check is to reach out to your local Sirv team.

It depends on your supports and funding, but we move quickly. Get in touch and we'll give you realistic timeframes for your situation — and privately funded support can often start sooner.

Contact your local Sirv team or complete a referral form. We'll have a chat about what you need, answer your questions and put a plan in place — no obligation.
